Diagnostic-Driven After-School Tutoring for K–12 Schools

Every student gets a diagnosis before they get a lesson plan.

No generic worksheets, no one-size-fits-all pacing. Every Forma Academics session is built around what a student actually needs next, with regular visibility into how it's going.

What Makes Forma Different

Built for real gaps, not general grade level

Diagnostic-first

Every student is assessed before and after each program, so growth is measured against their own starting point, not a generic benchmark.

Custom curriculum

Built for the specific students being served across all three programs, not off-the-shelf material.

Certified & background-checked

Every instructor completes a LiveScan/DOJ background check and TB clearance before working with students.

Bilingual staff

Spanish-speaking instructors available across programs, and required for Dual Immersion placements.

Small, focused groups

Low student-to-instructor ratios keep instruction genuinely individualized, not just labeled that way.

Flexible make-ups

Missed sessions get rescheduled, so a single absence doesn't mean a student falls behind on their plan.

Programs

Three programs, each built on its own diagnostic

Every program starts by finding out exactly where a student is. Then it teaches to that, not to the grade on their roster.

Math Tutoring

Diagnostic

Forma's own diagnostic curriculum, built to align with IXL, pinpoints exactly which standards a student has and hasn't mastered.

Curriculum

Custom Forma Academics math curriculum, not off-the-shelf material, delivered in small groups by certified instructors.

ELPAC / LTEL Support

Diagnostic

A custom in-house assessment covers reading, writing, speaking, and listening separately, rather than a single combined score.

Curriculum

Built specifically to strengthen ELPAC performance and support reclassification for long-term English learners.

Dual Immersion

Diagnostic

A custom in-house assessment aligned to the Estrellita scope and sequence.

Curriculum

Spanish speaking and listening fluency instruction using the Estrellita curriculum, taught by bilingual staff.

How It Works

Diagnose. Teach. Report. Re-assess.

01

Diagnose

Every referred student completes a diagnostic before instruction begins.

02

Teach

Small-group sessions, 2–3x per week, using a curriculum built around real gaps.

03

Report

Site leadership receives bi-weekly updates, so there is never a wait until the end to know how it's going.

04

Re-assess

A post-program diagnostic measures real growth against each student's own starting point.

For Schools & Districts

Forma Academics runs on-site at partner schools after school hours, and is built for Title I and high-need sites, at any grade level, that need real, visible growth, not just seat time.

  • Every instructor background-checked (LiveScan/DOJ) and TB-cleared before stepping into your school
  • Programs scoped to your site's actual need: Math, ELPAC/LTEL, or Dual Immersion
  • Flexible make-up scheduling so absences don't derail a student's plan
